Year 7 2023 CSEF Uniform Package Application Form
State Schools’ Relief (SSR) is a not-for-profit organisation that works closely with government schools across Victoria to ensure that every student, regardless of their background, can fully participate in education. Implementing the Year 7 CSEF Uniform Package, SSR has improved the lives of thousands of vulnerable children, by providing assistance with school uniforms and footwear.
Year 7 Explore students for 2023 that are recipients of the Camps, Sports and Excursion Fund (CSEF) will automatically qualify for the 2023 Year 7 CSEF Uniform Package. For more information on CSEF, see here: https://www.education.vic.gov.au/about/programs/Pages/csef.aspx

What’s in the Package?
The 2022 package contains the following items:
- 1 x Polo Top OR Shirt
- AND
- 1 x Trousers OR Shorts OR Summer Dress
- AND
- 1 x Pair of Shoes
PE items are NOT included as part of this package.
Cost to Families
Where SSR cannot provide uniform pieces directly to schools, families will receive a voucher that subsidises the cost of uniform items. Vouchers must be redeemed at PSW Uniform Shop – for location and trading hours, see their website: https://www.psw.com.au/schools/hampton-park-secondary-college.html.
Families receiving vouchers will be required to make a co-contribution payment towards the cost of the uniform pieces, due at the time of purchase. The voucher will list each subsidised uniform item and the maximum subsidy amount. To find out the amount required to cover the gap for each uniform item, please visit the PSW website.
Please note: Vouchers can only be used to purchase items listed, and do have an expiry date, so must be redeemed before then to be valid.
How Do I Apply for the Uniform Package?
This form has been developed by HPSC to record student uniform requests. Please complete the form once for each of your children attending the 2023 Year 7 Explore program.
Once applications have been approved and processed by State School Relief, the uniform items and vouchers will be sent to the college. At this time, families will be notified to collect their pack from HPSC.
